By electronically signing and submitting this release form above, I hereby voluntarily grant the irrevocable and unrestricted right and permission to Equal Opportunity Schools and its representatives, agents, employees, or affiliates, to use my name, testimonial, directory information (including class, school, district, city, and state), likeness in any photographs, video footage, and/or other digital media (“photos”) that I provide at any time for any and all marketing and promotional purposes. 

I also understand that the name, testimonial, directory information, and photos may be used in various media outlets, including but not limited to print publications, promotional flyers, education materials, derivative works, websites, social media, and advertisements. I waive the right to inspect or approve the final product or to the eventual use it might be applied. I agree that all such names, testimonial, directory information, and photos, and any reproductions thereof, and all plates, negatives, recording tape, and digital files are and shall remain the property of the Company. 

I acknowledge that I will not receive any payments, compensation, royalties, or other consideration for the use of the name, testimonial, directory information, and photos. 

I agree to release, hold harmless, and forever discharge Equal Opportunity Schools, its representatives, agents, employees, or affiliates, from any and all claims, causes of action, demands, damages, and liabilities arising out of or in connection with the use or distribution of the name, testimonial, directory information, and photos, including but not limited to any claims for invasion of privacy, appropriation of likeness or defamation. 

I have read this release form and understand and agree to all its content and terms. 

I certify that I am at least 18 years of age, and this release form is signed voluntarily and under no duress.
